Please consult a school calendar when completing this form to avoid scheduling a field trip during holidays or in-services. Complete one form for each group, filling out all the fields.

Please schedule your visit at least three weeks in advance. Request forms are processed in the order in which they are received. We will do our best to give all groups their first choice of days and times whenever possible. We will confirm your visit through mail, email, or a telephone call.
